ABSTRACT

El presente estudio tiene como objetivo identificar los conocimientos, actitudes y prácticas (CAP) relacionados con la ingesta de sal/sodio en niños, desde la perspectiva de los cuidadores. Este, es un estudio de corte transversal en el que participaron 2997 cuidadores que asistieron a una sesión educativa sobre alimentación saludable realizada en el marco del proyecto "Acciones Complementarias del Programa de Alimentación Escolar y Maná Infantil" de la Gobernación de Antioquia, liderado por la Escuela de Nutrición y Dietética de la Universidad de Antioquia, entre agosto y septiembre de 2015. Al recolectar la información, se obtuvo que un alto porcentaje de encuestados refieren no conocer la diferencia entre sal y sodio (79,1%) y desconocen la cantidad de sodio contenida en los alimentos que consumen sus hijos; no obstante, consideran importante limitar la ingesta de sal en los niños. Finalmente, se identifica un alto número de personas dispuestas a disminuir la ingesta de sal en sus hijos, pero no sucede lo mismo cuando se hace referencia al sodio, debido a que se requiere un conocimiento más especializado y específico para tomar decisiones relacionadas con el contenido de este mineral en los alimentos, dado que está oculto.


The present study aims to identify the knowledge, attitudes and practices related to salt / sodium intake in children from the perspective of caregivers. We conducted a crosssectional study involving 2997 caregivers who attended an educational session on healthy eating carried out within the framework of the project "Complementary Actions of the School Feeding Program and Children's Mana" carried out by the Government of Antioquia and led by the School of Nutrition and Dietetics of the University of Antioquia between August and September 2015. When collecting the information, it was found that a high percentage of persons surveyed reported not knowing the difference between salt and sodium (79.1%) and not knowing the amount of sodium contained in their child's foods; however, they considered the limiting of salt intake important for their children. Finally, a high percentage of participants were willing to reduce the salt intake of their children, but this was not the case with regard to sodium, because more specialized and specific knowledge is required to make decisions related to the content of this mineral in food, since it is hidden.

Abstract Background: Seasoning is one of the recommended strategies to reduce salt in foods. However, only a few studies have studied salt preference changes using seasoning. Objectives: The aim of this study was to compare preference for salty bread, and if seasoning can change preference in hypertensive and normotensive, young and older outpatients. Methods: Outpatients (n = 118) were classified in four groups: older hypertensive subjects (OH) (n = 32), young hypertensive (YH) (n = 25); older normotensive individuals (ON) (n = 28), and young normotensive (YN) (n = 33). First, volunteers random tasted bread samples with three different salt concentrations. After two weeks, they tasted the same types of breads, with seasoning added in all. Blood pressure (BP), 24-hour urinary sodium and potassium excretion (UNaV, UKV) were measured twice. Analysis: Fisher exact test, McNamer's test and ANCOVA. Statistical significance: p < 0.05. Results: Systolic BP, UNaV, and UKV were greater in HO and HY and they had a higher preference for saltier samples than normotensive groups (HO: 71.9%, HY: 56% vs. NO: 25%, NY; 6%, p<0.01). With oregano, hypertensive individuals preferred smaller concentrations of salt, with reduced choice for saltier samples (HO: 71.9% to 21.9%, and HY: 56% to 16%, p = 0.02), NO preferred the lowest salt concentration sample (53.6% vs. 14.3%, p < 0.01), and NY further increased the preference for the lowest one (63.6% vs. 39.4%, p = 0.03). Conclusions: Older and younger hypertensive individuals prefer and consume more salt than normotensive ones, and the seasoned bread induced all groups to choose food with less salt. Salt preference is linked to hypertension and not to aging in outpatients.


Resumo Fundamento: Adicionar temperos aos alimentos é umas das estratégias recomendadas para diminuir a quantidade de sal nos alimentos. No entanto, poucos estudos investigaram alterações na preferência ao sal através do uso de temperos. Objetivos: O objetivo deste estudo foi comparar a preferência pelo pão salgado, e até que ponto o uso de temperos pode alterar as preferências dos indivíduos hipertensos e normotensos, pacientes ambulatoriais jovens e idosos. Métodos: Os pacientes ambulatoriais (n = 118) foram classificados em quatro grupos: idosos com hipertensão (IH) (n = 32), jovens hipertensos (JH) (n = 25); indivíduos idosos normotensos (IN) (n = 28), e jovens normotensos (JN) (n = 33). Primeiro, os voluntários provaram amostras aleatórias de pão com três diferentes concentrações de sal. Após duas semanas, eles provaram os mesmos tipos de pão, porém acrescidos de temperos. A pressão arterial (PA), e a excreção urinária de sódio e potássio de 24 horas (UNaV, UKV) foram medidas duas vezes. Análise: Teste exato de Fisher, teste de McNemar e teste ANCOVA. Significância estatística: p < 0,05. Resultados: A PA sistólica e a excreção urinária de sódio e potássio foram maiores nos grupos IH e JH, e eles tiveram maior preferência por amostras mais salgadas quando comparados com os grupos de normotensos (IH: 71,9%, JH: 56% vs. IN: 25%, JN; 6%, p < 0,01). Quando o orégano foi adicionado, a preferência dos indivíduos hipertensos foi pelas amostras com menores concentrações de sal, com uma diminuição da escolha por amostras mais salgadas (IH: 71,9% a 21,9%, e JH: 56% a 16%, p = 0,02); o grupo IN preferiu a amostra com a concentração de sal mais baixa (53,6% vs. 14,3%, p < 0,01) e no grupo JN aumentou ainda mais o número de indivíduos com preferência pela amostra com concentrações mais baixas de sal (63,6% vs. 39,4%, p = 0,03). Conclusões: Os idosos e jovens hipertensos preferem e consomem mais sal do que os normotensos, e o pão adicionado de tempero ajudou todos os grupos a escolher alimentos menos salgados. A preferência ao sal está ligada à hipertensão e não à idade nos pacientes ambulatoriais.

ABSTRACT

A transição alimentar e nutricional promoveu mudanças significativas no padrão alimentar da população. O estudo teve como objetivo avaliar os teores de sal e sódio ofertados no almoço de um restaurante universitário, de acordo com as recomendações da Organização Mundial de Saúde e Guia Alimentar para a População Brasileira. Como resultados verificou-se que o per capita de sal obtido para o almoço foi de 4,76 g (95,2 % da recomendação diária da OMS e do Guia Alimentar para a População Brasileira). A média de sódio de todas as preparações ofertadas no almoço foi de 1.877, 24 mg de sódio (93,86 % da recomendação diária segundo a OMS). Concluiu-se com o estudo que os teores de sódio encontrados nas preparações do almoço do restaurante universitário foram superiores à recomendação do Guia Alimentar para a população brasileira e da OMS.

ABSTRACT

O grande desafio da indústria cárnea é o desenvolvimento de produtos mais saudáveis e que não afetem negativamente as características de qualidade. O objetivo desse trabalho foi produzir hambúrgueres de frango com redução de sal e avaliar sua coloração. Para tanto, foram elaborados os seguintes tratamentos: controle (T1 - com quantidade regular de sal) e T2, T3 e T4 com redução de 15%, 30% e 45% de sal, respectivamente, o qual foi substituído por mix de ervas e especiarias. Os hambúrgueres foram analisados quanto a coloração antes e após a cocção. A coloração de hambúrgueres apresentou mais alterações antes da cocção. A coloração vermelha reduziu com a diminuição de sal, antes do cozimento. A redução de sal a partir de 30% diminuiu a luminosidade e a intensidade de amarelo.

ABSTRACT

O objetivo deste estudo foi desenvolver um patê de siri utilizando sal de ervas como alternativa para a redução do cloreto de sódio. Para isso, foram preparadas duas formulações, a formulação F1 com sal de ervas, com redução de 80% de cloreto de sódio e a formulação F2 com sal marinho. Para caracterização do produto, foram determinados a composição centesimal, inocuidade e a composição nutricional. Os resultados obtidos mostraram que a formulação F1 apresentou menores teores de sódio e potássio, bem como, menor valor calórico. Com relação a composição nutricional, o patê de siri oferecerá ao consumidor uma alternativa de produto com elevado teor de proteína e baixa caloria, característico de um alimento saudável. Quanto aos padrões microbiológicos, ambas as formulações se encontraram dentro dos parâmetros estabelecidos pela legislação vigente. De fato, a substituição de cloreto de sódio por "sal de ervas", é uma excelente alternativa para a redução do teor de sal nos alimentos.

Introducción. La hipertensión enmascarada (HE) en niños se define por registros de presión arterial normal en la consulta y elevados fuera de ella. El monitoreo ambulatorio de presión arterial de 24 horas (MAPA) permite su diagnóstico. Existe poca información sobre su prevalencia en nuestra población. Objetivo. Estimar la prevalencia de HE en niños con factores de riesgo para el desarrollo de hipertensión arterial. Material y método. Estudio prospectivo, observacional, corte transversal. Se incluyeron en forma consecutiva pacientes asistidos en el Hospital General de Niños P. de Elizalde entre 1/7/2015 y 1/12/2016, de 5 a 11 años, con presión arterial normal y, al menos, un factor de riesgo para hipertensión arterial. Se realizó MAPA durante 24 horas (SpaceLabs 90207/90217). Muestra estimada: 110 pacientes. Se contó con las aprobaciones pertinentes. Resultados. Se incluyeron 110 pacientes, edad de 8,7 ± 1,8 años, 60 mujeres. Tiempo de MAPA: 23,18 ± 1,8 horas. Tenían antecedentes neonatales 23 pacientes; todos tenían, al menos, un antecedente personal (los más frecuentes, ingesta aumentada de sal y obesidad); 101 tenían, al menos, un antecedente familiar. El MAPA permitió identificar a 10 pacientes con HE (9,1%; IC 95%: 5,1-15,9); 7 con hipertensión nocturna aislada (6,4%; IC 95%: 3,1-23,5) y 28, prehipertensión (25,4%; IC 95%: 18,2-34,3). Los 10 pacientes con HE fueron 7 varones, 9 obesos y con, al menos, un antecedente familiar. Conclusión. La prevalencia de HE en niños con factores de riesgo de desarrollar hipertensión arterial fue cercana al 10%.


Introduction. Masked hypertension (MH) in children is defined as normal office blood pressure values and high values outside the clinical setting. The 24-hour ambulatory blood pressure monitoring (ABPM) is helpful for diagnosis. There is little information on MH prevalence in our population. Objective. To estimate the prevalence of MH in children with risk factors for arterial hypertension. Material and method. Prospective, observational, cross-sectional study. Patients seen at Hospital General de Niños "P. de Elizalde" between July 1st, 2015 and December 1st, 2016, aged 5-11 years, with normal blood pressure and at least one risk factor for arterial hypertension were included in consecutive order. A 24-hour ABPM was done (SpaceLabs 90207/90217). Estimated sample: 110 patients. All relevant authorizations were obtained. Results. One hundred and ten patients aged 8.7 ± 1.8 years were included; 60 were girls. ABPM duration: 23.18 ± 1.8 hours. Twenty-three patients had neonatal history; all had at least one factor corresponding to personal history (the most common ones were increased salt intake and obesity); 101 had at least one factor corresponding to family history. ABPM helped to identify 10 patients with MH (9.1%; 95% confidence interval --1;CI--3;: 5.1-15.9); 7 had isolated nocturnal hypertension (6.4%; 95% CI: 3.1-23.5) and 28 had prehypertension (25.4%; 95% CI: 18.234.3). Among the 10 patients with MH, 7 were boys, 9 were obese and had at least one factor corresponding to family history. Conclusion. The prevalence of MH in children with risk factors for arterial hypertension was close to 10%.

ABSTRACT Sodium chloride is traditionally used as a food additive in food processing. However, because of its high sodium content, NaCl has been associated with chronic diseases. Margarine is a popular product that is used in several preparations, but it includes high sodium content; therefore, it is among the products whose sodium content should be reduced. Thus, the objective of this study was to produce margarines with reduced sodium content prepared using a salt mixture. The following 4 margarine formulations were prepared: Formulation A (control - 0% sodium reduction), Formulation B (20.8% less sodium), Formulation C (33.0% less sodium) and Formulation D (47.4% less sodium). The low sodium formulations were produced using a salt mixture consisting of NaCl, KCl, and monosodium glutamate at different concentrations. The margarines were evaluated using an acceptance test and descriptive tests: time-intensity and temporal dominance of sensations. The mixture used is a good alternative for preparing low sodium margarine because the low sodium formulations feature equal salinity and do not produce a strange or bad taste. Furthermore, it may be possible to prepare margarines with up to 47.4% less sodium and that are acceptable to consumers.

ABSTRACT

Elevated salt intake induces changes in the extracellular matrix collagen, leading to myocardial stiffness and impaired relaxation. Resistance training (RT) has been used as a remarkably successful strategy in the treatment of heart disease. Therefore, the aim of this study was to investigate the effects of RT on preventing pathological adaptation of the left ventricle (LV) induced by salt overload. Male Wistar rats (10 weeks old) were distributed into four groups (n=8/group): control (CO), control+1% salt (CO+SALT), RT and RT+1% salt (RT+SALT). The RT protocol consisted of 4×12 bouts of squat training, 5/week for 8 weeks, with 80% of one repetition maximum (1RM). Echocardiographs were analyzed and interstitial collagen volume fraction (CVF) was determined in the LV. The 1RM tests in the RT and RT+SALT groups increased 145 and 137%, respectively, compared with the test performed before the training program. LV weight-to-body weight ratio and LV weight-to-tibia length ratio were greater in the RT and RT+SALT groups, respectively, compared with the CO group. Although there was no difference in the systolic function between groups, diastolic function decreased 25% in the CO+SALT group compared with the CO group measured by E/A wave ratio. RT partially prevented this decrease in diastolic function compared with the CO+SALT group. A 1% salt overload increased CVF more than 2.4-fold in the CO+SALT group compared with the CO group and RT prevented this increase. In conclusion, RT prevented interstitial collagen deposition in LV rats subjected to 1% NaCl and attenuated diastolic dysfunction induced by salt overload independent of alterations in blood pressure.

ABSTRACT

Abstract: The Less Salt, More Life program was the first voluntary salt reduction initiative in Argentina. This article analyzes the perspectives of the stakeholders involved in this voluntary agreement between the Ministry of Health and the food industry to gradually reduce sodium content in processed foods. This exploratory case study used a qualitative approach including 29 in-depth interviews with stakeholders from the public and private sectors and identified the role of the different stakeholders and their perceptions regarding the challenges encountered in the policy process that contribute to the debate on public-private partnerships in health policies. The article also discusses the initiative's main challenges and controversies.


El programa Menos Sal, Más Vida fue la primera iniciativa voluntaria para la reducción de la sal en Argentina. Este artículo analiza las perspectivas de los representantes del sector público y privado involucrados en este acuerdo voluntario, entre el Ministerio de Salud y la industria alimentaria, para reducir gradualmente el contenido de sodio en las comidas procesadas. Este estudio de caso se basó en una aproximación cualitativa, incluyendo 29 entrevistas en profundidad, con las partes interesadas del sector público y privado e identificó el papel de los mismos y sus percepciones respecto a los desafíos enfrentados durante el proceso, con el fin de contribuir al debate de las colaboraciones público-privadas en políticas de salud. El artículo también discute los principales desafíos y controversias.


O programa Menos Sal, Mais Vida foi a primeira iniciativa voluntária para reduzir o teor de sal em produtos alimentícios na Argentina. O artigo analisa as perspectivas dos atores envolvidos nesse acordo voluntário entre o Ministério da Saúde e a indústria alimentícia para reduzir gradualmente o teor de sódio nos alimentos processados. O estudo de caso exploratório utilizou uma abordagem qualitativa com 29 entrevistas em profundidade com representantes dos setores público e privado, e identificou o papel dos diversos atores e suas percepções quanto aos desafios enfrentados no processo político, contribuindo para o debate sobre parcerias público-privadas em políticas de saúde. O artigo também discute os principais desafios e controvérsias dessa iniciativa.

Resumo: O objetivo do presente estudo foi avaliar o potencial impacto da redução do teor de sódio em alimentos processados no consumo médio de sódio na população brasileira. Um total de 32.900 participantes do primeiro Inquérito Nacional de Alimentação (2008-2009), com 10 anos e mais de idade, que forneceram dados de dois dias de consumo foram avaliados. As metas de redução de sódio pactuadas pelo Ministério da Saúde em 2010 e 2013 foram utilizadas como referência para determinar o teor máximo de sódio em 21 grupos de alimentos processados. Os resultados indicam que as metas de redução de sódio em alimentos processados têm pequeno impacto no consumo médio de sódio na população brasileira. Em 2017, a redução média esperada é de 1,5%, ficando os valores de consumo médio de sódio ainda acima do limite máximo recomendado de 2.000mg/dia. Portanto, dificilmente será possível alcançar a redução necessária no consumo de sódio no Brasil a partir de acordos voluntários nos moldes dos que aconteceram até o momento.


Resumen: El objetivo del presente estudio fue evaluar el potencial impacto de la reducción del contenido en sodio en alimentos procesados en el consumo medio de sodio de la población brasileña. Un total de 32.900 participantes de la primera Encuesta Nacional de Alimentación (2008-2009), con 10 años y más de edad, proporcionaron datos sobre dos días de consumo, que fueron evaluados. Las metas de reducción de sodio, indicadas por el Ministerio de Salud en 2010 y 2013, fueron utilizadas como referencia para determinar el contenido máximo de sodio en 21 grupos de alimentos procesados. Los resultados indican que las metas de reducción de sodio en alimentos procesados tiene un pequeño impacto en el consumo medio de sodio en la población brasileña. En 2017, la reducción media esperada es de un 1,5%, quedando los valores de consumo medio de sodio todavía por encima del límite máximo recomendado de 2.000mg/día. Por tanto, difícilmente será posible alcanzar la reducción necesaria en el consumo de sodio en Brasil, a partir de acuerdos voluntarios en los términos de los que se han ido sucediendo hasta el momento.


Abstract: This study aimed at assessing the potential impact of the reduction of sodium content in processed foods in the average salt intake in the Brazilian population. A total of 32,900 participants of the first National Dietary Survey (NDS 2008-2009), age 10 years and older who provided information about food intake over two days were evaluated. The sodium reduction targets established by the Brazilian Ministry of Health in 2010 and 2013 were used as the reference to determine the maximum content of sodium in 21 groups of processed food. The results show that sodium reduction targets in processed food have small impact in mean Brazilian population intake of salt. For 2017, the expected mean reduction is of 1.5%, the average sodium intake being still above the recommended 2,000mg/day maximum. Therefore, it will hardly be possible to reach the necessary reduction in salt intake in Brazil from volunteer agreements like the ones made so far.

ABSTRACT

High salt intake is related to an increase in blood pressure and development of hypertension. However, currently, there are no national representative data in Brazil using the gold standard method of 24-h urine collection to measure sodium consumption. This study aimed to determine salt intake based on 24-h urine collection in a sample of 272 adults of both genders and to correlate it with blood pressure levels. We used a rigorous protocol to assure an empty bladder prior to initiating urine collection. We excluded subjects with a urine volume <500 mL, collection period outside of an interval of 23-25 h, and subjects with creatinine excretion that was not within the range of 14.4-33.6 mg/kg (men) and 10.8-25.2 mg/kg (women). The mean salt intake was 10.4±4.1 g/day (d), and 94% of the participants (98% of men and 90% of women) ingested more than the recommended level of 5 g/d. We found a positive association between salt and body mass index (BMI) categories, as well as with salt and blood pressure, independent of age and BMI. The difference in systolic blood pressure reached 13 mmHg between subjects consuming less than 6 g/d of salt and those ingesting more than 18 g/d. Subjects with hypertension had a higher estimated salt intake than normotensive subjects (11.4±5.0 vs 9.8±3.6 g/d, P<0.01), regardless of whether they were under treatment. Our data indicate the need for interventions to reduce sodium intake, as well the need for ongoing, appropriate monitoring of salt consumption in the general population.

ABSTRACT

El objetivo del presente estudio fue caracterizar la alimentación de los adolescentes uruguayos y los aspectos sociodemográficos que influyen en la misma. Los datos provienen de la Encuesta Nacional de Adolescencia y Juventud del año 2008 la cual trabajó con 2.943 casos, muestra representativa del total de adolescentes del Uruguay. Las características de la alimentación estudiadas fueron: consumo de frutas y verduras, bebidas azucaradas tipo cola, comidas rápidas, agregado de sal a las comidas servidas en la mesa y tiempos de comida compartidos con alguno de sus padres. Estas variables también se estudiaron de forma agregada para determinar un patrón de comportamiento alimentario. Se obtuvo que 89% de los adolescentes no alcanzó la recomendación de 5 porciones de frutas y verduras al día, 50% consumió diariamente bebidas azucaradas, 24% ingirió comidas rápidas 2 o más veces por semana y 13% agregó sal a todas las preparaciones. También se encontró que 31% no compartió diariamente tiempos de comida con sus padres. Al agregar las variables estudiadas, se observó que 58% se alimentó inadecuadamente, existiendo un mayor riesgo entre los adolescentes de mayor edad (p<0,01), quienes presentaban mayores ingresos familiares (p<0,05) y aquellos que residían en el interior del país (p<0,05). Se concluye que la alimentación de los adolescentes se caracterizó por la escasa ingesta de frutas y verduras, el frecuente consumo de bebidas azucaradas tipo cola, de comidas rápidas y agregado de sal a las comidas servidas, lo que la definió como inadecuada. Esto se observó principalmente entre adolescentes de mayor edad, mejor situación económica y residentes en el interior del país.


The aim of this study was to characterize the diet of Uruguayan adolescents and demographic aspects that influence it. The data come from the National Survey of Adolescents and Youth in 2008 which worked with 2,943 cases, representative sample of all adolescents in Uruguay. The characteristics of feeding studied were: consumption of fruits and vegetables, soft drinks, fast food, added salt to meals served at the table and meal times shared with a parent. These variables were also studied in aggregate to determine a pattern of eating behavior. It was found that 89% of adolescents did not meet the recommendation of 5 servings of fruits and vegetables a day, 50% consumed daily soft drinks, fast foods ingested 24% 2 or more times per week and 13% added salt at all preparations. We also found that 31% did not share mealtimes with parents daily. By adding the variables studied, we observed that 58% were inadequately fed, and is higher among older adolescents (p <0.01), who had higher household income (p <0.05) and those who were not residing in the capital (p <0.05). It is concluded that feeding adolescents was characterized by inadequate intake of fruit and vegetables, frequent consumption of soft drinks, fast food and adding salt to served meals, which defined it as inadequate. This was mainly observed in older adolescents, better economic situation and residents within the country.

OBJETIVOS: Descrever a prevalência de consumo elevado de sódio autorreferido em adultos e comparar resultados das capitais brasileiras e Distrito Federal, coletados pela Pesquisa Nacional de Saúde (PNS) e pelo Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel), ambos de 2013. MÉTODOS: estudo descritivo utilizando dados da PNS e Vigitel, estimando prevalências e intervalos de confiança (IC95 por cento). RESULTADOS: segundo a PNS, 14,2 por cento (IC95 por cento:13,6 por cento -14,7 por cento) dos adultos referiram consumo elevado de sal, com prevalência maior em homens (16,1 por cento; IC95 por cento:15,3-16,9), indivíduos de 18-29 anos de idade (17,7 por cento; IC95 por cento: 16,2-19,2), com Ensino Superior completo (17,3 por cento; IC95 por cento: 15,6-19,0), residentes na área urbana (14,8 por cento; IC95 por cento: 13,6-14,7) e na macrorregião Sul (18,2 por cento; IC95 por cento: 16,8-19,7); no total das capitais, não houve diferenças entre PNS (15,0 por cento; IC95 por cento: 14,2-15,8) e Vigitel (16,0 por cento; IC95 por cento: 15,3-16,6), porém observaram-se diferenças significativas para Rio Branco e Aracaju. CONCLUSÃO: encontrou-se prevalências elevadas, semelhantes em ambos estudos, reforçando a importância do Vigitel para monitoramento.

CONCLUSIÓN: las tasas de prevalencia fueron altas y la semejanza de valores encontrada refuerza la importancia de Vigitel para ese monitoreo.


OBJECTIVE: To describe self-reported high sodium consumption prevalence in adults and compare results in Brazilian capitals and the Federal District based on data from the 2013 National Health Survey (PNS) and from the 2013 Surveillance System for Risk and Protective Factors for Chronic Diseases by Telephone Survey (Vigitel). METHODS: this was a descriptive study using PNS and Vigitel data, estimating prevalence and confidence intervals (95 per cent CI). RESULTS: PNS data indicates that 14.2 per cent (95 per cent CI:13.6 per cent -14.7 per cent) of adults reported high sodium consumption. It was higher in men (16.1 per cent; 95 per cent CI:15.3-16.9), people aged 18-29 (17.7 per cent; 95 per cent CI:16.2-19.2), those with higher education (17.3 per cent; 95 per cent CI:15.6-19.0), living in urban areas (14.8 per cent; 95 per cent CI: 13.6-14.7), and in Southern Brazil (18.2 per cent; 95 per cent CI:16.8-19.7). When comparing capitals, there was no statistical difference between PNS and Vigitel, although statistical difference was found in Rio Branco and Aracaju. CONCLUSION: similar rates of high prevalence were found in both studies, reinforcing Vigitel's important monitoring role.

Introducción. La hipercalciuria idiopática (HI) predispone al desarrollo de infección del tracto urinario (ITU); sin embargo, hay escasa información local sobre dicha asociación. Nuestros objetivos fueron estimar la prevalencia de HI en niños con ITU y evaluar si esta difería según la presencia o no de reflujo vesicoureteral (RVU). Complementariamente, analizamos la asociación entre HI y la ingesta de sal. Población y métodos. Determinamos la calciuria a pacientes menores de 18 años con ITU estudiada (ecografía y cistouretrografía miccional) y ausencia de causas secundarias de hipercalciuria. Consideramos HI al cociente calcio/creatinina > 0,8; 0,6; 0,5 y 0,2 en niños de 0-6 meses, 7-12 meses, 12-24 meses y en los mayores de 2 años, respectivamente; e ingesta elevada de sodio, al cociente sodio/potasio urinario > 2,5. Resultados. En 136 pacientes (87 niñas, mediana de edad 3 años), la prevalencia de HI fue de 20%. Los pacientes con (n= 54) y sin (n= 82) RVU fueron similares en género, peso, talla, edad al diagnóstico y al momento del estudio, características clínicas (hematuria, nefrolitiasis, dolor cólico y recurrencia de ITU), antecedentes familiares de nefrolitiasis y en la prevalencia de HI (26% vs. 16%, p= 0,24). Los niños hipercalciúricos presentaron ingesta elevada de sodio más frecuentemente que los normocalciúricos (76% vs. 46%, p= 0,007). Conclusiones. La prevalencia de HI en niños con ITU fue alta (20%) y no difirió entre los pacientes con y sin RVU. Sería recomendable la búsqueda de HI en los niños con ITU, independientemente de la presencia o no de RVU.


Introduction. Idiopathic hypercalciuria (IH) predisposes to urinary tract infections (UTIs); however, there is scarce local information regarding such association. Our objectives were to estimate IH prevalence in children with UTI and to assess whether there were differences in relation to the presence or absence of vesicoureteral reflux (VUR). Additionally, the association between IH and salt intake was studied. Population and Methods. Calciuria was determined in patients younger than 18 years old on whom UTI had been studied (ultrasound and voiding cystourethrogram), and who had no secondary causes of hypercalciuria. IH was defined as a calcium to creatinine ratio of >0.8, 0.6, 0.5 and 0.2 in children aged 0 to 6 months old, 7 to12 months old, 12 to 24 months old and older than 2 years old, respectively; and a high sodium intake with a urinary sodium to potassium ratio of >2.5. Results. IH prevalence among 136 patients (87 girls, median age: 3 years old) was 20%. Patients with VUR (n= 54) and without VUR (n= 82) had similar characteristics in terms of sex, weight, height, age at diagnosis and age at the time of the study, and clinical features (hematuria, nephrolithiasis, colicky pain, and recurrent UTI), family history of kidney stone formation, and IH prevalence (26% versus 16%, p= 0.24). A high sodium intake was more frequently observed in children with hypercalciuria than in those with normal urine calcium levels (76% versus 46%, p= 0.007). Conclusions. IH prevalence in children with UTI was high (20%), with no differences observed between patients with and without VUR. As a recommendation, the presence of IH should be detected in children with UTI, regardless of VUR presence or absence.

ABSTRACT

The iodine nutritional status of a population is mainly measured by urinary iodine excretion (UI) and thyroid volume determined by ultrasound (US). The surveillance of nutritional iodine levels in the Mexican population has been insufficient. Our aim was to determine the UI in random samples from adults living in an urban area. We selected a sample of healthy individuals over the age of 18 that were students, physicians or administrative personnel at our Institution and had no known thyroid disease. Thyroid volume was determined by ultrasound in all volunteers as well as thyroid hormones and antithyroid antibodies and a urine sample was obtained. One hundred and two volunteers with a median age of 29, participated in the study. The group’s median UI was 221 μg/L,interquartile range (IQR)(135.0 to 356.8) and no differences were observed between genders: women had a UI of 218.0 μg/L IQR (129.0 a 351.0) vs. 223.0 μg/L IQR (138.0 to 374.0) in males, p 0.941. Excessive dietary iodine intake was established in 31.4% of all volunteers according to their UI, placing them at risk of thyroid dysfunction. It is fundamental to evaluate the national iodine nutritional status in Mexico. We discuss the current status of the nutritional state in the Americas, emphasizing that in Latin America and the observed tendency in the region to ingest a diet high in iodine.


El estatus nutricional del yodo en una población, principalmente es medido por la excreción del yodo urinario (UI) y el cálculo del volumen tiroideo por ultrasonido (US). La vigilancia nutricional del yodo en la población en México ha sido escasa. El objetivo fue determinar la UI en una muestra casual en adultos sanos que habitan en un área urbana. Se seleccionó una muestra de individuos mayores de 18 años, entre estudiantes, médicos y trabajadores administrativos de nuestra institución, sanos, sin enfermedad tiroidea conocida. A todos los voluntarios se les determinó el volumen tiroideo por US, hormonas y anticuerpos antitiroideos y se les solicitó una muestra de orina. Participaron 102 voluntarios, mediana de 29 años. La mediana de la UI del grupo fue 221 μg/L, rango intercuartílico (RIQ) (135.0 a 356.8), no hubo diferencias entre géneros, las mujeres tuvieron una UI de, 218.0 μg/L RIQ (129.0 a 351.0) vs 223.0 μg/L RIQ (138.0 a 374.0) p 0.941. El 31.4% de los voluntarios mostraron una ingestión excesiva de yodo en la dieta de acuerdo a la UI, lo cual los coloca en riesgo de sufrir disfunción tiroidea. Es necesario considerar la evaluación Nacional del estatus nutricional del yodo, en México. En el artículo se discute la situación actual del estatus nutricional en las Américas, haciendo énfasis en Latinoamérica y a la tendencia de la región a la dieta excedida en yodo.

ABSTRACT

O objetivo desta pesquisa foi investigar se existem diferenças na adição de sal à refeição pronta entre homens e mulheres. Foram incluídos 47.557 indivíduos de 18 a 64 anos, participantes do sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico, das 26 capitais e Distrito Federal, em 2006. As diferenças entre homens e mulheres foram analisadas pelo teste do qui-quadrado e as magnitudes da associação estimadas pelo Odds Ratio obtido por meio da regressão logística múltipla. A prevalência da adição de sal à refeição pronta foi 8,3%, sendo maior entre homens (9,8% vs 6,9%, p < 0,01). Após ajustes, a adição de sal à refeição pronta foi maior em indivíduos com a autoavaliação de saúde regular e ruim, relato de doença cardiovascular e que residiam na região Norte do Brasil. Indivíduos hipertensos relataram adicionar menos sal à refeição pronta. A escolaridade não foi associada à adição de sal. Homens adicionam mais sal à refeição pronta do que mulheres. Políticas públicas de saúde voltadas para a redução da ingestão de sal pela população devem levar em consideração a diferença de gênero no uso discricionário de sal e nos fatores que favorecem essas diferenças.


The scope of this research was to investigate the potential differences between men and women in the addition of salt to prepared food. The study included 47,557 individuals aged 18 to 64 participating in the Risk and Protection Factors for Chronic Disease Surveillance System by Telephone Interview carried out in 26 Brazilian state capitals and the Federal District in 2006. Differences between men and women were tested by the chi-square test and the association magnitudes between the dependent and independent variables were estimated by the Odds Ratio obtained by Multiple Logistic Regression analysis. The prevalence of the addition of salt to prepared food was 8.3%, being higher among men (9,8% vs 6,9%, p < 0.01). After adjustment, the addition of salt to prepared food was higher in individuals with self-rated fair to poor health, reporting cardiovascular disease and living in the North of Brazil. Hypertensive individuals reported addition of less salt to prepared food. Educational level was not associated with salt usage. Men add more salt than women. Public health policies aimed at reducing salt intake by the population should take into account the gender differences in salt intake and the factors that contribute to such differences.
